At KARMA PRIVATE HOME CARE, we understand how daunting and stressful it is to find a good caregiver that you can rely and trust your loved one with. One of the biggest cause of this stress is the lack of information. At KARMA PRIVATE HOME CARE, we respect and value our client “right to information” and in making sound decision fully informed on the processes and services we provide. When we receive your call or email for a free assessment appointment, our nurse care coordinator will visit you at your home and perform a need assessment, which will include the following process:
ASSESSMENT
Our care coordinator will do a head to toe assessment of the client and collect subjective and objective data from the client.
NEEDS IDENTIFICATION
Based on the assessment of the client, our nurse will identify the strength and limitation of the clients health status, and collaborate with the client to address those needs.
OUTCOME IDENTIFICATION
Based on the clients need assessment, our nurse and client will collaboratively develop a goal or outcome they want to achieve by specific time that is realistic and measurable.
PLANNING
Our nurse and the client mutually develops individualized care plan that reflect client value, clinical needs, and preferences to reach the identified outcome needs.
IMPLEMENTATION
After the care plan has been made, our caregiver will follow the individualized care plan on the daily basis to reach the clients care plan goals. Our caregiver will encourage support, and validate the client in taking agreed on action to achieve goals and expected outcomes through integrated, therapeutic intervention and communication strategies.
EVALUATION
Every month, our nurse and client will mutually evaluate attainment of expected outcome and survey each step of the process for appropriateness, effectiveness, adequacy, and time efficiency, modifying the plan as indicated by evaluation.
